Trinalgin: composition, indications, dosage, side effects.

It is an anti-inflammatory, analgesic, antipyretic agent. Metamizole inhibits the activity of COX, reducing the formation of bradykinins, endoperekisy, free radicals, some prostaglandins. Also inhibits the process of lipid peroxidation. Blocks the conduction of painful extra- / proprioceptive nerve impulses along Burdaha / Gaulle tufts, increases the threshold for the excitability of pain-sensitive centers in the thalamus, and increases heat transfer. Pitofenon has a direct myotropic antispasmodic action. Fenpiverin has m-anticholinergic effects, has antispasmodic effects.

Composition and release form
Active ingredients: sodium metamizole (500 mg), pitofenone hydrochloride (2 mg), fenpiverin bromide (0.02 mg).

Trinalgin is produced in the form of injection solution.

Indications
Trinalgin is used for symptomatic treatment of smooth muscle spasms, pain syndrome of different origin (with renal colic, spasms of the urinary tract, stomach and intestines, spastic dysmenorrhea, biliary dyskinesia, biliary colic, angina, myocardial infarction, migraine-like headache).

Also used in the period before and after diagnostic studies (in order to eliminate the spasm of the investigated organs).

If necessary, it can be prescribed to reduce body temperature in case of catarrhal, infectious and inflammatory diseases.

Contraindications
Trinalgin do not apply:

  • with intolerance to the components present in the composition of this drug;
  • with a significant violation of the functional state of the liver / kidneys;
  • with agranulocytosis;
  • with leukopenia;
  • With anemia of any origin;
  • with cytostatic or infectious neutropenia;
  • with rhinitis, conjunctivitis, bronchospasm in the past, arising from the use of nonsteroidal anti-inflammatory drugs;
  • with bronchial asthma;
  • with hepatoporphyria;
  • if acute surgical pathology is suspected;
  • in case of intolerance to pyrazolone derivatives (butadion, tribuzon, antipyrine);
  • with hereditary hemolytic anemia;
  • with glaucoma;
  • with tachycardia;
  • in case of kidney diseases (pyelonephritis, glomerulonephritis);
  • at kolaptoidny states;
  • with prostatic hypertrophy;
  • with stenosis of the alimentary canal;
  • at megacolon;
  • children up to 15 years.

Use during pregnancy and lactation
Do not apply.

Dosage and administration
Trinalgin parenterally applied.

The duration of treatment is usually 1-3 days.

The solution is administered intravenously / intramuscularly.

For acute pain, 2–5 ml is administered by slow intravenous injection. Repeated administration is possible in 6–8 hours. Maximum – 10 ml / day.

Overdose
Symptoms: vomiting, sweating, feeling of dry mouth, accommodation disturbance, drowsiness, arterial hypotension, confusion of consciousness, hepatic / renal failure, convulsions.

Treatment: symptomatic therapy is carried out in the required amount.

Side effects
With intramuscular administration, pain is possible at the injection site, with intravenous administration, lowering of blood pressure. Rarely observed hyperthermic syndrome, hypertensive crisis.

Disorders of the digestive system: nausea, constipation, pain / burning in the abdomen.

Immune system disorders: allergic reaction of varying severity (urticaria, angioedema, conjunctivitis, Stevens-Johnson syndrome, bronchospastic syndrome, Lyell’s syndrome, anaphylactic shock).

Blood disorders: thrombocytopenia, leukopenia, agranulocytosis.

Disorders of the urinary system: nephrotoxicity, proteinuria, oliguria / anuria, interstitial nephritis, urine staining in red.

Cardiovascular disorders: AV block, ventricular premature beats, reduction of cardiac output.

Nervous system disorders: drowsiness, sweating, breathing problems.
