Ethacizine

Analogues

Novocainamide ‑Darnitsa, Lidocaine Zdorovye, Meksaritm, Propanorm, Ritmonorm.

pharmachologic effect

Ethacizine is a class I antiarrhythmic drug. It has a pronounced and prolonged antiarrhythmic effect.

Indications for use

Ventricular and supraventricular extrasystole, paroxysms of atrial fibrillation and flutter, ventricular and supraventricular tachycardia.

Dosage and administration

Inside, regardless of food intake, starting with 50 mg 2-3 times a day. In case of insufficient clinical effect, the dose is increased to 50 mg 4 times a day (200 mg) or 100 mg 3 times a day (300 mg).

When a stable antiarrhythmic effect is achieved, maintenance therapy is carried out in individually selected minimally effective doses.

Some patients require combined treatment with Etacizine and beta- ‑adrenergic blockers to achieve a stable antiarrhythmic effect.

Side effects

Depending on the size of the dose, in order to avoid the occurrence of side effects, maximum doses of the drug should not be prescribed.

In individuals with individual hypersensitivity, allergic reactions are possible. There may be a stop of the sinus node, AV ‑blockade, a violation of intraventricular conduction, a decrease in myocardial contractility, a decrease in coronary blood flow, arrhythmia. Arrhythmogenic effect, the probability of which is highest after a myocardial infarction and in other types of cardiac pathology, leads to a decrease in the contractile function of the myocardium and the development of heart failure.

Contraindications

Hypersensitivity to the drug or excipients; severe conduction disturbances, impaired intraventricular conduction; severe hypertrophy of the myocardium of the left ventricle; the presence of postinfarction cardiosclerosis; cardiogenic shock; severe arterial hypotension; chronic heart failure II and III class; severe violations of the liver and / or kidneys; During pregnancy and breastfeeding; childhood; simultaneous reception of MAO inhibitors; simultaneous use of antiarrhythmic drugs IC – (Morazizin, Propafenone, Allapinin) and ‑class IA (Quinidine, Procainamide, Disopyramide, Aymalin); the appointment of Etatsizin to patients with any form of cardiac arrhythmias in combination with blockades of the conduction through the bundle of His – Purkinje fibers.

Overdose

Symptoms. Prolongation of the P- Q interval and expansion of the QRS complex, an increase in the amplitude of the T waves, bradycardia, sinoatrial and AV ‑blockade, asystole, paroxysms of polymorphic and monomorphic ventricular tachycardia, decreased myocardial contractility, persistent decrease in blood pressure, dizziness, blurred vision, headache, gastrointestinal disorders.

Treatment. symptomatic; for ventricular tachycardia, do not use antiarrhythmic drugs IA– and IC ‑classes; sodium bicarbonate is able to eliminate the expansion of the QRS complex, bradycardia and arterial hypotension.
