pharmachologic effect

Antianginal analgesic of the group of organic nitrates, dilating venous vessels. When using the drug, there is an increase in exercise tolerance in patients with angina pectoris. In heart failure, the drug contributes to the unloading of the myocardium by reducing the preload. Reduces pressure in the pulmonary circulation.

Indications for use

Prevention of angina attacks; treatment of chronic heart failure; pulmonary hypertension.

Dosage and administration

Inside, 1 tablet 1-2 times a day. The average daily dose is 20-100 mg, the maximum daily dose is 120 mg (in case of emergency, the dose can be increased to 240 mg 1 time per day).

Tablets should be swallowed whole with a small amount of liquid.

Side effects

Blurred vision, headache, dizziness and feeling of weakness, restlessness; arterial hypotension, sensation of heat, reflex tachycardia, reddening of the skin of the face, sometimes with a sharp decrease in blood pressure – increased symptoms of angina pectoris; dry mouth, nausea, vomiting; in some cases – skin rash, itching, exfoliative dermatitis.

Contraindications

Shock, collapse, arterial hypotension, acute myocardial infarction with low filling pressure of the left ventricle, hypertrophic obstructive cardiomyopathy, increased intracranial pressure, angle-closure glaucoma with high intraocular pressure, hypersensitivity to nitrates, simultaneous use of sildenafil, toxic pulmonary edema. Childhood, breastfeeding period.

Overdose

Symptoms . Reflex tachycardia, weakness, dizziness, nausea, vomiting, diarrhea, hyperemia, headache, fear, orthostatic hypotension, in severe cases – cyanosis.

Treatment. Symptomatic – oxygen inhalation, elevated position of the lower extremities, in case of taking a large number of tablets – gastric lavage. In the presence of methemoglobinemia, which is manifested by cyanosis, in / in the slow introduction of methylene blue at a dose of 1 mg / kg.
