pharmachologic effect

Local anesthetic with moderate anesthetic activity and a broad spectrum of therapeutic action.

Indications for use

Local and infiltration anesthesia, therapeutic blockade.

Method of administration and dosage

With local anesthesia, the dose of the drug depends on the concentration, the nature of the surgical intervention, the route of administration, the condition and age of the patient.

When perirenal blockade in perirenal fat adults administered 50-70 ml of 0.5 % (5 mg / ml) p – pa .   

For infiltrative anesthesia the following maximum dose set ( for adults ): the first single dose at the beginning of operation – 0.75 g (i.e., 150 ml of 0.5 % p – pa ).    

Later during each hour of operation – no more than 2 g (i.e., 400 ml of 0.5 % p – pa ).    

Side effects

Headache, dizziness, drowsiness, weakness, restlessness, loss of consciousness, convulsions, trismus, tremors, visual and auditory disturbances, nystagmus, equine tail syndrome, respiratory muscle paralysis, motor and sensory block, pain recurrence, persistent anesthesia, allergic reactions.

Contraindications

Hypersensitivity to the drug, myasthenia gravis, arterial hypotension, purulent process at the injection site, urgent surgical interventions accompanied by acute blood loss, pronounced fibrotic changes in tissues, age up to 18 years.

Overdose

Symptoms   Pale skin and mucous membranes, dizziness, nausea, vomiting, irritability, cold sweats, tachycardia, decreased blood pressure, tremors, seizures, apnea, infants, respiratory depression, sudden cardio – vascular collapse.

Treatment . General resuscitation measures, which include inhalation of oxygen, if necessary – carrying out mechanical ventilation. If seizures last more than 15–20 seconds, they are stopped by intravenous thiopental (100–150 mg) or diazepam (5–20 mg).   

With arterial hypotension and / or depression of the myocardium, ephedrine (15-30 mg) is administered intravenously , in severe cases detoxification and symptomatic therapy is used.
